Job Description:
CASE MANAGER - Per diem
The Case Manager is responsible for a collaborative process that assesses plans, implements, coordinates, monitors and evaluates the options and services required to meet an individual’s health care needs. The goal of the case manager at Anna Jaques Hospital is to provide quality and cost effective outcomes for the patient populations it services. Core components of this role involve concurrent utilization review and coordination and implementation of discharge planning needs in a time efficient manner according to patient’s individual needs. The case manager is responsible for adhering to policies on confidentiality, advocating for patient rights and working in accordance with regulatory agencies to maintain regulatory compliance.
Qualifications:
Graduate of an accredited nursing program
MA RN license or license eligible
3-5 years of experience in an acute care care setting, preferably in case management
FLSA Status:
Non-Exempt
