The Specialty Care Coordinator identifies opportunities to better coordinate patient care, new initiatives and enhancements of existing processes. Will assist in managing escalations and working with patients, practice staff, providers and payors to keep care local, while focusing on timeliness and high quality care. The Specialty Care Coordinator works extensively via telephone, EMRs and other communication mechanisms, coordinating clinical information to secure needed clinical care, and ensure connection with established ongoing caregivers or specialists. Maintains strong relationships with BILHPN providers/provider practices and jointly develops standard triage guidelines for optimal transitions.

Job Description:

The Specialty Care Coordinator’s Responsibilities are:

· Serves as a resource for the BILH system who can coordinate specialty care across BILHPN. Collaborates with employed and affiliated physicians and patients to secure timely and medically appropriate access to care within the BILH Performance Network.

· Work with Medical Directors to educate PCPs and patients on in-network care and the BILHPN referral guidelines. Communicates access challenges and patient concerns to the Medical Directors as appropriate.

· Registers and schedules appointments for patients with providers who can see the patient in a timely manner. Communicates back to patients all appointment details.

· Appropriately documents referral outcomes to promote team awareness and ensure patient safety in accordance with department standards. Coordinates with the referral team on tracking program metrics as appropriate.

· Collaborates with Referral Management leaders on program evaluation and improvement initiatives.

· Ensures complete and accurate patient demographic and current insurance information.

· Assemble information concerning patient’s clinical background and referral needs and provides appropriate clinical information to specialist.

· Review details and expectations about the referral with patients.

· Assist patients in problem solving potential issues related to health care systems.

· Ability to address problems that are highly varied, complex and often non-recurring.

· Identifies and utilizes cultural and community resources. Establish and maintain relationships with identified service providers (i.e. interpreter services).

· Ensure that referrals are addressed in a timely manner.

· Reduce misdirected referrals, helping patients get to the right providers.

· Improve referral processes, reducing burdens on providers and staff.

Qualifications:

· High school diploma and 10 years health care experience

· Associate’s degree and 5 years health care experience

· Bachelor’s degree and 3 years health care experience

· Clinical experience a plus

· Certificate/license in healthcare related field a plus

· Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.

· Strong communication, both written and verbal
