About the Job

Become a member of the Winchester Hospital Case Management Team! Position requires the ability to work on any type of hospital unit, but primarily within the Ed in a role that supports effective leveling as well as safe readmission prevention. Position requires deep knowledge of community health resources, specifically home health and post-acute facility placement. Requires the ability to develop strong working relationships with external constituents as well as internal providers and the entire care team. Position is critical to the success of hospital-based initiatives and will be multi-faceted based on organizational needs including the ability to ensure the provision of high quality, safe patient care.

Required:

BSN is required

EXPERIENCE

Required

  • At least three (3) years of home health and/or post-acute care experience.

Preferred

  • Previous Case Management experience

Other Skills/Knowledge

Required

  • Excellent interpersonal skills and the ability to work collaboratively with a provider staff as well as the rest of the case management team.

  • For safety and quality reasons, must be able to read, write and communicate effectively in English with patients, visitors, vendors, and fellow members of the hospital team.

  • Computer skills

LICENSES, REGISTRATIONS, CERTIFICATIONS

Required

  • Current license to practice professional nursing from MA Board of Registration

  • Case Management certification

Shift

Full Time - 40 hours including one weekend per month
